当事業所は「笑顔を紡ぎ、暮らしを織る」を理念に掲げ、心ふれあう在宅支援サービスを提供しております。

ご高齢の方の訪問介護。障がいを持つ方の居宅介護を通じて、利用者様のご自宅での暮らしを支え、安心と豊かな生活を実現を日々努めています。

We provide warm and caring home support services.
Through home visits for elderly people and in-home care for people with disabilities,
we support their daily lives at home, helping them feel safe and live comfortably.

ご利用者様が住みなれた地域で生活することを支えます。サービス提供地域は、私の地元「東成区」を中心に近隣の「城東区」「生野区」「天王寺区」となります。

長年、病院勤務で培った医療の経験を活かし、ご利用者様やそのご家族が、どのような状況にあっても安心して生活を続けられるよう、質の高いケアと支援を提供してまいります。

We support people so they can keep living in their familiar neighborhood.
Our service area includes my hometown, Higashinari-ku, and nearby areas like Joto-ku, Ikuno-ku and Tennoji-ku.
With many years of experience working in hospitals,
we use our medical knowledge to give high-quality care and support.
No matter the situation, we want to help users and their families live safely and with peace of mind.

サービス内容

「ひばり」では、訪問介護員等（ホームヘルパー）がご自宅にお伺いし、利用者様の日常生活を支えるための様々な介護サービスを提供しております。

当事業所のホームヘルパーは、利用者様一人ひとりのニーズに応じたきめ細やかなサービスを心がけています。

具体的には、食事や排せつの介助を行う「身体介護」、調理や掃除、洗濯などの日常的な家事をサポートする「生活援助」など、ご自宅での快適な生活をサポートするための幅広いサービスを提供しています。

At Hibari, our home helpers (visiting caregivers) go to the homes of users and provide many kinds of care services to support their daily lives.
Our caregivers always try to give careful and personal support that matches each person’s needs.
We offer a wide range of services, such as:
・Body care, like helping with meals and using the toilet
・Daily life support, like cooking, cleaning, and doing laundry
These services help people live comfortably in their own homes.

訪問介護計画書または居宅介護計画等の作成

まず、ご利用者ならびに相談支援従事者（またはケアマネ）の意向や心身の状況等のアセスメントを行います。
援助の目標に応じて、具体的なサービス内容を定めた手順書を作成し、この手順書を元に訪問介護計画書・居宅介護計画書などを作成します。
この計画書に、ご利用者様の同意を得てから日々の支援に入ります。心身の状況等の変化に伴い、更新されカスタマイズされます。

First, we assess the user’s needs, wishes, and physical and mental condition, together with the user and their care manager or support staff.
Based on the care goals, we create a step-by-step guide that shows the details of the services.
Using this guide, we make a home-visit care plan or in-home care plan.
After the user agrees to the plan, we start daily support.
If the user’s condition changes, we update and customize the plan as needed.

管理者兼サービス提供責任者　ご挨拶

人情味あふれる大阪が好きで、東京から移住。あっという間に20年が経ちました。

居宅介護等に8年ほど関わり、利用者様の「生きづらさ」を痛感。次に放課後等デイサービスで10年間、障がいを持つお子さんの支援に従事し、ケイパビリティの涵養に尽力。2025年、再び在宅支援に復帰。
